What gynecomastia surgery is, and what it isn't
Every week I see men who have been carrying this for years. One man is twenty-two and has been wearing compression shirts since sixth form. Another is forty-one and hasn't taken off his shirt at the beach in a decade. A few have spent money on supplements, chest workouts, and devices that promised to fix it. None of it worked, because none of it was ever going to work. Gynecomastia is glandular tissue. You cannot exercise it away. For most patients — Grade I and II — I remove the fatty tissue through a 3–4mm port site using liposuction, then excise the glandular disc behind the nipple through a small incision along the lower areola edge. The incision is placed in the natural colour change of the areola. From the front, nothing is visible. The procedure takes 60 to 90 minutes as a daycare surgery. You go home the same day. Most men are back at a desk within a week. Here is what gynecomastia surgery is NOT. It is not reversible by the gym. I have patients who are competitive bodybuilders with visible gynecomastia. Muscle mass does not reduce glandular tissue. If you have been told to just work out more, you have been given the wrong advice. It is not the same as chest fat. Pseudogynecomastia — fat-only chest enlargement — is treated with liposuction alone. True gynecomastia has a firm glandular disc behind the nipple that does not respond to anything except surgical excision. A clinical exam or a WhatsApp photo assessment distinguishes the two. It is not risky to fix. This is one of the most reproducible procedures in plastic surgery. A thorough pre-op assessment manages the anaesthetic risk. About 70% of final result visible. Vest comes off during the day. Light lower-body gym resumes. No chest pressing yet.

Recurrence is uncommon once glandular tissue is removed. The fat component will not return if your weight stays stable. If the original cause was hormonal or medication-related, addressing that before surgery reduces the chance of any residual tissue returning.
Gynecomastia surgery is a well-established procedure with a low complication rate. Temporary swelling, bruising, and numbness are expected. Serious complications — infection, asymmetry, haematoma — are uncommon. A thorough pre-operative assessment is done before any surgery is recommended.
